Incident Overview

On 17 September 2025, a fatal accident occurred at Equinor's Mongstad facility during the lifting of heat exchangers. A crane hook unintentionally caught on a bolt, resulting in a worker being trapped between two heat exchangers. Despite lifesaving efforts, the worker's life could not be saved.
