Lilly's Omvoh achieves multidimensional corticosteroid‑free remission in UC and Crohn's at one year

Key highlights
  • 27.7% of Omvoh-treated ulcerative colitis patients achieved comprehensive disease control at one year versus 14% with placebo.
  • 20.4% of Omvoh-treated Crohn's patients achieved corticosteroid‑free deep remission at one year versus 2% with placebo.
  • Comprehensive disease control (UC) combines clinical, endoscopic and histologic remission, fecal calprotectin ≤250 μg/g and ≥12 weeks corticosteroid‑free; corticosteroid‑free deep remission (Crohn's) combines CDAI clinical remission, endoscopic remission and ≥12 weeks corticosteroid‑free.
  • Analyses included patients with prior biologic failure (35.3% in UC, 48.6% in Crohn's); achieving the composite endpoints was strongly associated with IBDQ remission.

Overview

Post‑hoc analyses from the LUCENT (UC) and VIVID (Crohn's) Phase 3 programs show that mirikizumab (Omvoh) enabled multidimensional, corticosteroid‑free treatment outcomes at one year. Results were presented at UEG Week 2026 and included patients with prior biologic failure.

Ulcerative colitis — LUCENT‑2

Comprehensive disease control is a composite outcome combining clinical remission, no corticosteroid use for at least 12 weeks before Week 52, histologic remission and fecal calprotectin ≤250 μg/g. In patients who responded at Week 12 and continued treatment, 27.7% achieved comprehensive disease control on Omvoh versus 14% on placebo; among those with prior biologic failure (35.3% of the cohort) the rates were 23.4% versus 9.4%. Of Omvoh‑treated patients who achieved comprehensive disease control, 91.1% also achieved IBDQ remission; these patients reported less bowel urgency and greater work productivity improvements.

Crohn's disease — VIVID‑1

Corticosteroid‑free deep remission combines clinical remission by CDAI, endoscopic remission and no corticosteroid use for at least 12 weeks before Week 52. At one year, 20.4% of Omvoh‑treated patients achieved this outcome versus 2% with placebo; among patients with prior biologic failure (48.6%), 15.7% on Omvoh achieved it versus none on placebo. Among Omvoh responders, 88.1% who met the composite endpoint also achieved IBDQ remission.

Ongoing research

Lilly is evaluating mirikizumab in combination studies (eltrekibart, zotemtegrast, an FXR agonist) and in COMMIT‑UC/COMMIT‑CD trials and pediatric programs (including NCT04844606 and NCT05509777) to explore induction efficacy, maintenance and broader populations.
